Overview

In this role, the RN will manage patient care through assessment, care coordination, and discharge planning in an acute care setting. The position requires working closely with a multidisciplinary team to ensure optimal patient outcomes. Candidates should have at least two years of acute care case management experience and be prepared to meet department needs, including weekend work.
